**Need Weekend On-Call --> Available all day for potential visits on either Saturday or Sunday or BOTH!
***We pay hourly plus on-call bonus

Responsibilities:
- Conduct patient assessments to identify patient needs and develop personalized care plans.
- Deliver skilled nursing care to patients in their homes, focusing on pain management, wound care, and medication management.
- Collaborate with physicians and other healthcare professionals to ensure comprehensive patient care.
- Provide education and support to patients and their families to promote healthy behaviors and self-care.


Qualifications:
- Current licensure as a Registered Nurse in the state of practice.
- Experience working in home health.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Familiarity with electronic medical records (EMRs) and home health care standards.


Pay: Starting at $38 per hour
