Registered Nurse (RN) Case Manager / RN Supervisor – Home Care
BrightStar Care is seeking an experienced RN Case Manager / RN Supervisor to provide clinical oversight, case management, and leadership for our home care teams.
Schedule & Details
Monday–Friday | 8:30 AM – 4:30 PM
No weekends
Local travel required
~75% field / 25% office
Rotating on-call schedule
Key Responsibilities
Lead SNV and infusion visits
Oversee admissions, reassessments, supervisory visits, and case management
Clinical lead for CCBW program (Mayo partnership)
QA review of non-skilled documentation and caregiver notes
Manage infusion intake, scheduling, orders, and POCs
Support PDN admissions, reassessments, and occasional shift coverage
Assist with chart audits, training support, and DON administrative tasks
- Active Florida LPN license
- 1+ year nursing experience
