What You’ll Do:

• Perform in-home physical therapy evaluations and develop personalized care plans.
• Deliver skilled therapy services focused on improving strength, balance, gait, and functional mobility.
• Educate patients and family members on home exercise programs and safe mobility practices.
• Collaborate with our interdisciplinary BrightStar team including nurses, OTs, HHAs, and case managers.
• Document patient progress and visit notes using our mobile-friendly EMR system.
• Ensure patient safety and quality outcomes aligned with BrightStar’s core standards and compliance protocols.

Requirements:

• Active and unrestricted Massachusetts Physical Therapist license
• Graduate of an accredited PT program (DPT or MPT)
•  CPR certification
• Minimum 1 year of PT experience preferred (new grads welcome with supervision)
• Reliable vehicle and valid driver’s license
• Ability to work independently and as part of a care team
