Licensed Practical Nurse (LPN) – Home Health


Salary: $35–$45 per hour
Bonuses: Referral Bonus + Performance Bonus

Responsibilities:

•  Administer medications and treatments as prescribed by healthcare providers 
•  Monitor and document patient vital signs and overall health status 
•  Assist with daily living activities such as bathing, grooming, and meal preparation 
•  Educate patients and families on managing health conditions and care plans 
•  Coordinate with RNs, therapists, physicians, and other healthcare professionals 
•  Maintain accurate and timely documentation using EHR systems 
•  Provide emotional support and companionship 
•  Follow all safety and health regulations in home care settings 

Requirements:

•  Active and valid LPN license in Massachusetts 
•  Minimum 1 year of experience in home health or related healthcare setting 
•  Current BLS certification 
•  Reliable transportation and willingness to travel to patient homes 
•  Proficiency in electronic health records (EHR) 
•  Strong communication and interpersonal skills 
•  Compassion and commitment to quality patient care
