Key Responsibilities
- Provide comprehensive and compassionate physical therapy services in patients’ homes in accordance with physician orders
- Perform initial evaluations and ongoing assessments of patients’ functional abilities
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and quality of life
- Deliver skilled therapeutic interventions, exercises, and patient/caregiver education
- Document patient progress accurately and in a timely manner per company and regulatory standards
- Collaborate with nurses, caregivers, physicians, and interdisciplinary teams to ensure coordinated care
- Maintain a safe, supportive, and professional therapy environment at all times
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist (PT/RPT) license in Massachusetts
- Prior experience in home care of 2 years, skilled nursing, or sub-acute rehab preferred but not required
- Strong communication, interpersonal, and clinical documentation skills
- Ability to work independently and manage a full-time caseload
- Reliable transportation and ability to travel within assigned service areas
